File Reference: MER-94-BCS-0417 1. PURPOSE This memorandum documents the completion of continuity review activities performed under the referenced contract. Review was conducted in accordance with standing segment review procedures and applicable regional distribution requirements. This file records the personnel, access, and sign-off information associated with the period of performance. No exceptions to standard procedure were recorded. 2. SCOPE OF REVIEW Review covered scheduled and unscheduled broadcast segments distributed within the Meridian County service area during the period of performance. Segments were assessed for continuity compliance and, where applicable, retained in the segment archive under the governing protocol. Coordination with regional distribution was performed as directed by the contracting agency. Standard staffing was maintained across day, evening, and overnight shifts for the duration of the review window. Facility access during the period is recorded in the attached logs (see Appendices A and B). 3. FINDINGS Review activities were completed within the period of performance. All reviewed segments were processed under standard procedure and dispositioned accordingly. No further action items remain open under this contract. The engagement is closed. 4. ATTACHMENTS

NOTICE 94-081 — SCHEDULED RECALIBRATION, SEGMENT REVIEW EQUIPMENT Issued 09/06/1994 — Regional Operations Annual recalibration of segment review and duplication equipment will proceed per the standing maintenance calendar. Units will be rotated out of service in groups of two. No interruption to review activities is anticipated. Direct scheduling questions to Regional Operations. NOTICE 94-088 — AWARD OF CONTRACT, HARLAN RIDGE SERVICE AREA Issued 10/03/1994 — Contracts Office CSG has been awarded a broadcast continuity contract covering the Harlan Ridge service area (Kentucky). Period of performance begins January 1995. Staffing assignments to follow through the ordinary channel. The company thanks the Contracts Office for its work on this engagement. NOTICE 94-094 — RETIREMENT, D. KACZMAREK (DUPLICATION UNIT) Issued 11/07/1994 — Personnel After nineteen years of service, Donald Kaczmarek of the Duplication Unit will retire effective the end of November. A small reception will be held at the regional office. Colleagues wishing to contribute to the farewell card should see Personnel by 11/21. NOTICE 94-102 — CLARIFICATION REGARDING BRACKETT FACILITY INCIDENT Issued 11/28/1994 — Office of the Regional Director In response to inquiries concerning the recent incident at the Brackett facility (Meridian County engagement), the company confirms that the matter is under county review and that the cause has been identified as an electrical fault in the east wing. All requested records have been provided in full. No further inquiries will be answered while the review is pending. In the interest of transparency, the following witness statement is provided as supporting record:

NOTICE 94-107 — HOLIDAY OPERATING SCHEDULE Issued 12/12/1994 — Regional Operations The regional office will operate on a reduced schedule from 12/23 through 01/02. Overnight review shifts continue uninterrupted per contract requirements. Timesheets for the holiday period are due to Personnel no later than 01/06/1995. NOTICE 95-004 — RELOCATION, REGIONAL OFFICE Issued 02/13/1995 — Regional Operations Effective March 1995, the regional office will relocate to Suite 240, Commerce Park Drive. Mail and courier deliveries should be redirected accordingly. Telephone extensions are unchanged. Boxed files awaiting transfer should be labeled with unit and shelf designation.
